29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a new multi-family residence worksite located at 8600 block N Indiana Ave, KC MO -- employees engaged in installing siding were not using fall protection when working on the lower roof which was approximately 11' above ground level. This was documented on or about 1/16/2025.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a new multi-family residential worksite located at 8600 N Indiana Ave, KC MO -- employees engaged in installing siding were using an extension ladder to gain access to the lower roof. The extension ladder did not extend 3 feet above the upper landing surface. This exposed employees to fall hazards. This was documented on or about 1/16/2025.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:  At a new multi-family residential worksite located at 8600 block of N Indiana Ave, KC MO -- employees engaged in installing siding were using portable step ladders. The ladder were not in the open position. Instead, they were closed and leaned up against the building while on the lower roof. This exposed employees to fall hazards. This was documented on or about 1/16/2025.
