29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926.  On or about January 18, 2025, and at times prior thereto, employees accessed the second floor landing of a stairwell without fall protection while prepping hoses to pour concrete.
