29 CFR  1910.147(c)(4)(i):Procedures were not developed and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about December 10, 2024, the employer did not develop and utilize energy control procedures prior to allowing employees to perform servicing and maintenance activities on the MBO Folding machine.  Employees clearing jams on the MBO Folding machine were exposed  to crushing hazards from in-going nip points because the hazardous energy source(s) were not controlled.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
