Location: Job site;  On or about 01/27/2025, employees were observed working in the zone of danger beneath an active siding project where siding materials and equipment were not secured or protected from falling 16-feet and striking workers in the head on the ground who were not protected by hardhats.

29 CFR 1926.451(g)(1)(vii): An employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Location: Jobsite;  On or about 01/27/2025, employees were exposed to a fall of greater than 16-feet above the ground while working from a pump jack scaffold that was erected without a complete guardrail system or fall protection system to protect them.

Location: Job site;  On or about 01/27/2025, employees were allowed to work from a pumpjack scaffolding at a height of 16-feet where the poles/legs of the pumpjack scaffolding were not secured to the structure by rigid triangular bracing at both top and bottom.
