On or about: January 26, 2025 Location: 640 5th Avenue, New York, NY 10019  (a) Employees performed vinyl removal from a JLG 600AJ lift that extended into a traffic lane, without effective temporary traffic controls or equivalent measures. The employees were exposed to struck-by hazards due to the hazard of oncoming traffic making contact with the lift; the lift was struck by a bus
