29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  On or about January 29, 2025, during the installation of siding material at a job site located at Lot 186, 133 Benjamin Way, Tallmadge Ohio 44278; the employer did not ensure an employee working on a ladder jack scaffold was protected from a fall of approximately 14.4 feet by utilizing a fall protection system.

On or about January 29, 2025, during the installation of siding material at a job site located at Lot 186, 133 Benjamin Way, Tallmadge Ohio 44278; the employer did not ensure employees working on a ladder jack scaffold were protected from a fall of approximately 14.4 feet by ensuring the working surface platforms were not bridged one to another.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about January 29, 2025 at a work location located at Single family home; Lot 186, 133 Benjamin Way, Tallmadge Ohio 44278 the employer did not ensure that employees installing siding and removing snow from the roof on a residential structure were protected from falling from a height of approximately 22.6 feet above the ground. The supervisor was on site and actively directing the work of the exposed employees.
