29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:  a) RJ Hoffman & Sons, Inc., 1144 Buckwheat Valley Rd, Mt Pleasant Mills, PA - On or about January 27, 2025, the employer failed to report an injury that occurred on January 24, 2025, resulting in a four (4) finger amputation, which is not within the required twenty-four (24) hours to report the work-related incident.
