29 CFR  1926.20(b)(1):The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a) Jobsite located at 821 N 19th Street, Philadelphia, PA - On or about January 30, 2025, the employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during roofing activities, such as but not limited to fall hazards.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section:  a) Jobsite located at 821 N 19th Street, Philadelphia, PA - On or about January 30, 2025, the employer did not ensure an employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system. Employees were performing roofing activities on the roof of a four story residential building approximately 40 feet above ground level without the use of a fall protection system.
