29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  At 18170 Cottonwood Rd. Maple hill, Kansas, on or around October 25, 2024, employee was hospitalized following injury to his hand which happened while he was lifting the truck ramp to cross a threshold to unload the stone, and the employer did not report the hospitalization. On January 16, 2025, the employee's finger was amputated due to the injury. The employer failed to report the hospitalization and amputation. The employer reported the events on January 29, 2025.
