29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  (a) In front of building F - On or about January 28, 2025, the employer exposed employees to struck-by hazards, in that, the employer did not ensure employees wore protective helmets while cleaning up material on the ground under roofing activities above.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  (a) On the roof of building F - On or about the morning of January 28, 2025, the employer exposed employees to a fall hazard, in that, employees did not have fall protection while working on an approximately 6/12 slope roof with a 17' fall to ground distance.  (b) On the roof of building F - On or about the afternoon of January 28, 2025, the employer exposed employees to a fall hazard, in that, employees did not have fall protection while working on an approximately 6/12 slope roof with a 17' fall to ground distance.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) At the front of building F - On or about January 28, 2025, the employer exposed employees to a fall hazard, in that, the ladder used by employees for roof access did not extend at least 3 feet above the edge of the roof.
