29 CFR  1910.305(b)(2)(i): Each junction box in completed installations did not have a cover, faceplate, or fixture canopy:  a) Jobsite: 519 E Chelten Ave, PHILADELPHIA, PA - On or about January 31, 2025, the employer did not ensure junction box on the side of the Seybold Citation Paper Cutter had a cover or faceplate to prevent accidental contact with the exposed live conductor wires, thereby exposing employees to electrical shock hazards of approximately 110-120 volts.
