One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. At the establishment located at 5925 North Richmond Street, Appleton, Wisconsin: the employer did not provide machine guarding to protect operators and other employees in the machine areas from hazards during the following instances: a) On or about February 4, 2025, the employer did not protect employees from rotating part hazards created by the Fabbrica Macchine Bergamo Model Pegasus G SN: NH00150 13'6" band saw blade that was exposed beyond the point of operation; b) On or about February 4, 2025, the employer did not protect employees from rotating part hazards created by the DoAll Model C-912A SN: 368-83555 148" auto saw band saw blade that was exposed beyond the point of operation; c) On or about February 4, 2025, the employer did not protect employees from the point of operation and pinch point hazards on the Cincinatti Model 1810 SN: 25041 shear. The guarding was not adequate to prevent the operator and other employees from accessing the danger areas created by the shear and hold down. d) On or about February 4, 2025, the employer did not protect employees from rotating part hazards created by the Fabbrica Macchine Bergamo Model Triton SN: CO1272 8'10" band saw blade that was exposed beyond the point of operation; e) On or about February 4, 2025, the employer did not protect employees from rotating part hazards created by the Fabbrica Macchine Bergamo Model Manuali SN: NL00685 8'1" band saw blade that was exposed beyond the point of operation; f) On or about February 4, 2025, the employer did not protect employees from rotating part hazards created by the B&E Manufacturing Quick Set Model 500 contact wheel belt grinder that was exposed beyond the point of operation; g) On or about February 4, 2025, the employer did not protect employees from struck-by hazards created by an unguarded XForce CNC Plasma Table Model Captain 6120 Plasma Cutter carriage located on the production floor; h) On or about February 4, 2025, the employer did not protect employees from ingoing nip point hazards created by the Falls Metal Prodcuts Co. Model 111 SN: 1117 D-BUR-R bottom roller assembly that had no guarding.
