29 CFR  1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a) On or about February 4, 2025, and at times prior thereto, on the Back sheet gluing machine TS23-11-01serial number SC2475-2-C541-001, employees were cleaning the nozzles in side the machine without applying locks and/or tags to isolate the energy sources.
