29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  a) On or about January 15, 2025, near the front desk at the docks, the employer did not certify refresher training to a powered industrial vehicle (PIV) operator following an accident and before permitting the operator to resume forklift operations.  While one employee was operating a PIV, another employee driving a separate PIV crashed into it.
