29 CFR  1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 2 Maple Place, Yonkers - NY: Employees were exposed to fall hazards of approximately 18-feet while using an extension ladder to access a roof. The extension ladder that was used, was only extended approximately a foot and a half over the roof landing, on or about, January 15th, 2025.
