29 CFR  1926.451(g)(1)(i):Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system;  On or about February 10, 2025, employees performing construction activities from a ladder jack scaffold were not protected from a fall, exposing employees to a fall of 11.6 feet.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.454(a):The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:  On or about February 10, 2025, employees performing construction activities from a ladder jack scaffold were not provided training and unable to recognize and minimize the hazards associated with working from a scaffold.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
