29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. a) On or about May 5, 2025, the employer did not ensure that employees engaged in maintenance activities on the Saw #725 Do All SC-100A Circular Saw machine, received training in the recognition of applicable hazardous energy sources and the methods and means necessary for isolation and control. Employees were exposed to amputation and laceration hazards when performing blade changes. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about May 5, 2025, the employer did not ensure that employees implemented energy control steps prior to performing maintenance activities such as blade changes on the Do All Saw SC-100AP circular saw machine. Hazardous energy was not isolated prior to employees engaging in servicing and maintenance activities, exposing employees to laceration and amputation hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about January 29, 2025 employees were exposed to laceration and amputation hazards when operating the Do All Saw SC-100AP circular saw machine. Employees would operate machine with no guarding in place to prevent employees from getting their hands/fingers caught by the saw blade. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
