29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Equipment stores energy and manufacturer recommends waiting 10 minutes after disconnecting power before performing service. Equipment has more than one source of energy, electrical and pneumatic.   On or about February 12, 2025, on the south east side of the jobsite; the employer did not develop, document and utilize procedures for the control of hazardous energy while conducting troubleshooting operations on the Xinzhou Welder.

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about February 12, 2025, on the southeast side of the jobsite; Employee conducted  troubleshooting operations on the Xinzhou Welder, without electronic safety devices.

29 CFR  1904.39(a)(1):Basic Requirement.  Within eight (8) hours after the death of any employee as a result of a work-related incident, you must report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor.  On or about February 13, 2025, the employer failed to report an employee fatality to the Occupational Safety and Health Administration, U.S. Department of Labor within eight (8) hours.
