29 CFR  1910.157(g)(3): Employees who have been designated to use fire fighting equipment as part of the emergency action plan were not provided training in the use of the appropriate equipment:  On or about February 13, 2025, the employer did not ensure employees who were designated to use firefighting equipment were provided with training on the general principals of fire extinguisher use and the hazards involved with incipient state firefighting on an initial employment or annual basis. Employees were exposed to fire, burn, and respiratory fires in that training on the use of fire extinguishers was not provided. On or about February 13, 2025 employees were exposed to an electrical fire at an electrical receptable near bay door #27.

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Martin Motor Sports doing business as Martin Technologies located at 5655 Opportunity Drive Suite 6, Toledo, Ohio;  On or about February 13, 2025, the employer did not ensure that electrical equipment was installed and used in accordance with any instructions included in the listing or labeling. Employees were exposed to electrical shock and fire hazards in that a three-prong flexible extension cord and two T-Shaped outlet tap adapters were plugged in, in tandem, as a source of energy for equipment such as, but not limited to, a desk light, phone charger and a space heater, at a workstation near bay door #27.

29 CFR  1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Martin Motor Sports doing business as Martin Technologies located at 5655 Opportunity Drive Suite 6, Toledo, Ohio;  On or about February 13, 2025 the employer did not ensure that flexible cables were not used as a substitute for fixed wiring. Employees were exposed to electrical shock and fire hazards in that a flexible extension cord was used as fixed wiring to power electrical equipment such as, but not limited to, a space heater and a desk light at a work station near bay door #27.
