29 CFR  1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  Employee(s) are exposed to hazardous chemicals that can cause eye and skin irritation. This was documented on or about February 19, 2025, at 3232 H. Street, Omaha, Nebraska. Employee(s) are utilizing chemicals including but not limited to Barsol A-5442, Saati Remove HR2, and Xtend Ink Degradent 505 related to screen printing activities without  the use of required personal protection equipment.

29 CFR  1910.134(c)(2)(ii):The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user.  Employee(s) are exposed to hazardous chemicals that can cause respiratory health hazards. This was documented on or about February 19, 2025, at 3232 H. Street, Omaha, Nebraska. Employee(s) were authorized to use half facepiece respirators on a voluntary basis. The employer failed to implement the required elements of a respiratory protection program in accordance with this sub-part.

29 CFR  1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  Employee(s) are exposed to chemicals that can cause eye and skin irritation. This was most recently documented on or about February 19, 2025, at 3232 H. Street, Omaha, Nebraska. Employee(s) are utilizing chemicals including but not limited to Barsol A-5442, Saati Remove HR2, and Xtend Ink Degradent 505 related to Screen Printing. Instances of inadequate effective information and training included but were not limited to:  i.     Reclaiming screen printing screens. ii.    Remove stains from screens. iii.   Screen Printing.
