On or about October 23, 2024, at 305 Coronation Drive in Red Oak, Texas,  employees engaged in masonry activities utilized a green extension cord that is missing the ground pin exposing employees to electrical hazards.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer shall fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section.  On or about October 23, 2024 at 305 Coronation Drive in Red Oak, Texas, the employer did not ensure that employees using a Milwaukee masonry saw to cut bricks/stone were not protected from being exposed to silica dust with the use of engineering controls, work practices and respiratory protection.
