The result of no hazard assessment led to partial finger amputation of an employee that was operating the Bridgeport milling machine with gloves on.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment:  On or about September 10, 2024, the employer did not provide training to employees in the use of PPE to be worn /not worn when operating the Bridgeport milling machine when cutting gussets into aluminum tubing for safety rail production in the machine shop area of the facility located at1 High Tech Avenue, PAINESVILLE, OH 44077. This lack of training resulted in an employee wearing gloves while operating the mill, getting their hand caught in the spindle which led to a partial finger amputation and in patient hospitalization.

With no effective guarding in place, an employee wearing gloves while operating the mill, had unrestricted access to the rotating tooling assembly and point of operation, and getting their gloved hand caught in the spindle which led to a partial finger amputation and in patient hospitalization.
