29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) The Oxford Grain & Hay Co. located at 112 Railroad, Oxford, PA - On or about February 20, 2025, the employer did not ensure that the powered industrial truck (PIT) operator operating PITs such as, but not limited to, Toyota Electric Forklift Truck, Model SR1-BET30, Toyota Forklift Truck, Model 50-8FG0U, Hyster Forklift Truck, Model H50FT,  and Hyster Electric Pallet Jack, Model W45Z-HD  successfully completed training and evaluation to ensure the operator has the knowledge and skills needed to operate the powered industrial trucks safely, thereby exposing employees to caught in-between and struck-by hazards.  The Oxford Grain & Hay Co.  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 1476217, citation number 1, item number 1, group letter a, and was affirmed as a final order on December 15, 2020, with respect to a workplace located at 112 Railroad Ave, Oxford, PA 19363.
