29 CFR  1926.100(a):  Employees were not provided protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a) On February 21, employees working below other employees performing siding work were exposed to struck-by hazards in that they were not wearing hard hats.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.   a) On February 21, 2025, employees working from a pump-jack scaffold were exposed to the hazard of falling approximately 11.9 feet to the ground below.  No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  a) On February 21, 2025, employees were exposed to the hazard of falling between 8.6 and 15.6 feet while working without fall protection from a pitched roof.  No abatement certification or documentation required for this violation.
