OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from abrasive blaster tank rupture and/or explosion: a) On or about February 22, 2025, at the LA Water Storage Tank jobsite, employees operated a bulk abrasive blaster at pressures above Maximum Allowable Working Pressure (MAWP) of 125 psi. b) On or about February 22, 2025, at the LA Water Storage Tank jobsite, employees utilized a pressurized bulk abrasive blaster that was missing bolts and hinge pins from the blaster lid closure/hatch.
