29 CFR  1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  a) On or about February 25, 2025, at the shop facility: employees were exposed to an approximate 9 foot fall hazard while performing maintenance and repairs while on the roof of a passenger bus, in that fall protection was not used.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  a) On or about February 25, 2025, at the shop facility: employees were exposed to chemical health hazards, in that, spray bottles containing chemicals (Rubber and Plastic Dressing, Rapid Remover and Adhesive Remover, Magical Blue, Car Wash and Wax,  and  Ultimate Dressing Cleaner Degreaser) were not labeled.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) On or about February 25, 2025 at the shop facility: employees using Rubber and Plastic Dressing, Rapid Remover, Magical Blue, Car Wash/Wax,  and Ultimate Dressing Cleaner Degreaser, were exposed to chemical health hazards in that chemical specific training was not provided.
