29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:   a) On or about February 25, 2025, where employees were working on a roof greater than 6 feet high installing a roofing system without a fall protection system.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about February 25, 2025 employees at the south side of a new construction retail building the ladder employees used to access the roof to install roofing material was not extended 3 feet above the upper landing edge.
