29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a. At a temporary worksite, located at 6238 Mare Ave "Lot 628" Jacksonville FL:  On or about February 26, 2025, employees were exposed to the hazard of being stuck- by falling objects, while working below framing activities without being protected by protective helmets from overhead hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids, caustic liquids, or chemical gases or vapors and at times prior thereto, workers were exposed to a struck by hazard while cutting concrete with a saw, without the use and wear of protective eyewear:  a. At a temporary worksite, located at 6238 Mare Ave "Lot 628" Jacksonville FL:  On or about February 26, 2025, the employer exposed employees to struck-by hazards, in that protective eyewear was not utilized by employees while using nail guns and metal hammers for framing work.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. At a temporary worksite, located at 6238 Mare Ave "Lot 628" Jacksonville FL:  On or about February 26, 2025, the employer was exposing employees to a 10 to 11 foot fall hazard, in that the employees were not tied off when installing roof trusses.
