At the worksite, located at 230 Red Maple Pl, Brandon, FL 33510; On or about 2/26/25, The employer exposed employees to struck by hazards in that hard hats were not provided for employees conducting demolition operations on fire damaged walls, rooftops and overhead framing and trusses on the 2nd floor of an apartment building.

At the worksite, located at 230 Red Maple Pl, Brandon, FL 33510; On or about 2/26/25, The employer exposed employees to fall hazards of approximately 8 ft & 17 ft in that a fall protection method was not provided for employees standing on trusses & conducting demolition operations on fire damaged walls, rooftops and overhead framing and trusses.

At the worksite, located at 230 Red Maple Pl, Brandon, FL 33510; On or about 2/26/25, the employer exposed employees to fall and struck by hazards of approximately 8'8 & 17'5" in that an engineering survey was not provided for employees conducting demolition operations on fire damaged walls, rooftops and overhead framing and trusses on the 2nd floor of an apartment building.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:  a.) the employer exposed employees to fall hazards, of approximately 4 ft, in that, a Werner 6 ft A-frame ladder was not used in accordance with the manufacturers instructions as it was propped in the closed position, leaning against the wall while conducting demolition operations.
