29 CFR 1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about February 25, 2025, at a job site located at 34575 Aurora RD, Solon Ohio 44139; during the installation of masonry material on a commercial facility, the employer did not ensure two employees using an aerial lift were protected from fall hazards ranging from approximately 9 to 10 feet above the ground by utilizing a fall protection device.
