29 CFR  1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l):  a. On or about February 27, 2025, at the warehouse area.  Employees were exposed to struck by hazards when operating a Toyota LPG, Crown LPG and Crown electric power industrial trucks used to transport merchandise pallets without operator's certification of training and evaluation.   NOTE 1: CC1 Virgin Islands was previously cited for a violation of this occupational safety and health standard 1910.303(g)(1) or its equivalent standard, which was contained in OSHA inspection number 1745422, citation number 1, item number 2 and was affirmed as a final order on 10-24-2024, with respect to a workplace located at 5000 Estate Eliza Retreat, Christiansted, St. Croix, VI.  NOTE 2: Certification of abatement is not required for this citation item as abatement was provided and documented by the compliance safety and health officer prior to citation issuance.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a. On or about February 27, 2025, at the warehouse area. Two electrical service panels were not readily accessible as materials such as two 30-inch Commercial Electric fans were stored in front of the panels.   NOTE 1: CC1 Virgin Islands was previously cited for a violation of this occupational safety and health standard 1910.303(g)(1) or its equivalent standard, which was contained in OSHA inspection number 1745422, citation number 3, item number 1 and was affirmed as a final order on  10-24-2024, with respect to a workplace located at 5000 Estate Eliza Retreat, Christiansted, St. Croix, VI. 29 CFR  1910.101(b):  Section 3.3.4 Compressed Gas Association Pamphlet P1-1965, as adopted by 29 CFR 1910.101(b): charged and empty cylinders should be stored separately with the storage layout so planned that cylinders comprising old stock can be removed first with a minimum handling of other cylinders:   a. On or about February 27, 2025, at the warehouse area. Employees worked and walked by four empty 50-lbs carbon dioxide (UN 1013) gas cylinders that were not stored and secured to prevent being knocked over.    NOTE:  Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
