(a) On or about February 27, 2025, at Lot 38 Bastle Wynd, Condo Units 9&10, Port Washington, WI 53074, the employer did not ensure employees conducting installation of a new residential roof, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about February 27, 2025, at Lot 38 Bastle Wynd, Condo Units 9&10, Port Washington, WI 53074, employees installing a roof on a residential home roof with a 10:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 20 feet 2 inches from the roof eve to the ground below on the back (east) side of the home. (b) On or about February 27, 2025, at Lot 38 Bastle Wynd, Condo Units 9&10, Port Washington, WI 53074, employees installing a roof on a residential home roof with a greater than 6:12 pitch were not using fall protection. This exposed employees to a fall hazard of greater than 10ft from the roof eve to the ground below on the back (east) side of the home.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a) On or about February 27, 2025, at Lot 38 Bastle Wynd, Condo Units 9&10, Port Washington, WI 53074, the employer did not ensure employees installing a roof on a new residential home were properly wearing a full body harness that had a lifeline connected to the back dorsal D-Ring. Employees connected the lifeline to the front D-Ring of the body harness, exposing employees to a fall hazard of approximately 21 feet 2 inches from the roof eve to the ground below on the back (east) side of the home.

(a) On or about February 27, 2025, at Lot 38 Bastle Wynd, Condo Units 9&10, Port Washington, WI 53074, employees were accessing the roof to conduct roofing work from a ladder at the south side of the home which did not extend at least three (3) feet above the upper landing surface, exposing employees to a fall hazard of approximately 19 feet 11 inches. (b) On or about February 27, 2025, at Lot 38 Bastle Wynd, Condo Units 9&10, Port Washington, WI 53074, employees were accessing the roof to conduct roofing work from a ladder at the east side of the home which did not extend at least three (3) feet above the upper landing surface, exposing employees to a fall hazard of greater than 10ft.
