29 CFR  1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors:  a. On or about  February 27, 2025, at the break room area. An extension cord was used as permanent power source and was run along the floor and on the exit path, exposing employees to trip and fall hazards.   NOTE: Certification of abatement is not required as abatement was documented by the compliance safety and health officer within 24 hours of the inspection.

29 CFR  1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  On or about February 27, 2025, electrical service panels were not readily accessible as follows:  a. At the warehouse area.  A shopper handicap cart was stored in-front the electrical panel.   b. At the Little Caesars' kitchen storage area.  Six napkin boxes were stored in-front of two electrical panels.   c. At the Little Caesars' kitchen area. Cardboard boxes, one step ladder, and one 55-gallon garbage bin were stored in-front of two electrical panels.   NOTE 1: Kmart was previously cited for a violation of this occupational safety and health standard or its equivalent standard which was contained in OSHA inspection number 1670419, citation number 1, item number 1 and was affirmed as a final order on 12-11-2023, with respect to a workplace located at Kmart #3829, Tutu Park Shopping Mall, 26-A Tutu Park Mall, St. Thomas, VI.  NOTE 2:  Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer within 24 hours of the inspection.

29 CFR  1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points:  a. On or about  February 27, 2025, at the Little Ceasars' main entrance/exit area. There was a measurement of 17 inches wide along the exit access due to the storage of a wood pallet of large boxes of pizza ingredients which reduced the exit path, exposing employees working in the area.   NOTE: Certification of abatement is not required as abatement was documented by the compliance safety and health officer within 24 hours of the inspection.

29 CFR  1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a. On or about  February 27, 2025, at the break room area. Kitchen appliances such as a Kenmore microwave and Proctor Silex toaster oven were not directly connected to a permanently installed branch circuit receptacle. Two electrical extension cords were series connected and used to provide power to the appliances, exposing employees to electrical and fire hazards.   NOTE: Certification of abatement is not required as abatement was documented by the compliance safety and health officer within 24 hours of the inspection.
