Kennedy Blvd, Jersey City , New Jersey  Employees used  Werner extension ladders to access a residential roof to install new shingles without extending the ladder at least 3 feet above the landing surface. The access points of the roof were approximately 16 feet to the ground.  Violation observed 02/26/2025
