29 CFR  1926.651(f): A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation:  On or about February 28, 2025,  on the east side of the excavation, an employee operated a Caterpillar-Akashi 340 excavator without an adequate warning system.
