29 CFR 1910.179(j)(1)(ii):The employer did not perform frequent and periodic inspections as defined in paragraphs (a) and (b) of this section: a) 1407 Bulldog Dr Allentown - On or about March 3, 2025, Employees are utilizing a 5 ton overhead crane to install LNG tanks onto trucks. The employer has no inspection history for the overhead crane. b) 1407 Bulldog Dr Allentown - On or about March 3, 2025, Employees are utilizing a 5 ton crane with a spreader beam to hoist truck parts. The spreader beam does not indicate its maximum working load limit and the employer has no inspection history for the device. c) 1407 Bulldog Dr Allentown - On or about March 3, 2025, Employees utilize a 5 ton and 2 ton overhead crane to hoist truck parts. The overhead cranes do not have their load limits indicated so that they may be observed from the shop floor.

The task places the employees near the suspended while being lowered into place. The employer did not assess the workplace to determine overhead hazards were present, necessitating the use of head protection.
