29 CFR  1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  Location:  Production Areas  The employer has not established a lock out / tag out program for the servicing and maintenance of equipment  Instance A:  Cord-and-plug equipment is not being unplugged and maintained in control of the operator prior to servicing and maintenance tasks such as changing mixer blades.  Instance B:  Written LOTO procedures were not developed for equipment having more than 1 energy source - such as the cookie depositor.  Instance C:   LOTO procedures are not being inspected on a annual basis.

Production Areas  Rotating parts on mixers in the Mixer, Bread, and Pastry Rooms were not guarded.
