29 CFR 1910.133(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there was a hazard from flying objects. a) 2345 Milford Square Pike, Quakertown, PA (Shop Room) - On or about April 17, 2025, an employee who was tooling molds was wearing glasses that did not have side protective wings that would shield their eyes from flying metal from the process.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: a) 2345 Milford Square Pike, Quakertown, PA (Shop Room) - On or about April 17, 2025, an electrical box was situated behind a desk that prevented access for employees as per this section. b) 2345 Milford Square Pike, Quakertown, PA (Shop Room) - On or about April 17, 2025, two electrical switches controlling the two Vectrax machinery were obstructed by a steel drum, tubing, and a work cart, preventing access for employees as per this section. c) 2345 Milford Square Pike, Quakertown, PA (Outside Shop Room Wall) - On or about April 17, 2025, an electrical box was situated behind a Miller Deltaweld 452 and was obstructed by the equipment, preventing access for employees as per this section.
