On or about March 5, 2025, and at times prior, located at a residential construction worksite, employees conducted concrete pumping operations where the concrete pumping truck boom was being moved inside the minimum 10 feet distance of a 7.62kV energized line.

29 CFR  1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means.  On or about March 5, 2025, and at times prior, located at a residential construction worksite, an employee conducted concrete pumping operations within ten(10) feet of an energized overhead power line without having a designated observer to warn the operator when the boom was approaching the overhead power line.
