29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about May 14, 2025, the employer did not ensure that periodic inspections of energy control procedures were conducted at least annually to identify and correct any deviations or inadequacies. Employees engaged in set-up activities on the  Hamblet Sheeter without locking it out, exposing employees to crushing injuries if the machine had unexpectedly started.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about May 14, 2025, the employer did not ensure that that energy control procedures were utilized by employees performing set-up activities on the Hamblet  Sheeter  implemented  hazardous energy control application steps.  Employee(s) were exposed to un-expected start up or movement of the Hamblet Sheeter, resulting in exposure to caught-between hazards from machine parts that could result in amputations.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
