29 CFR  1910.134(e)(1): The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace:  a) Keehn Service Corporation located at or near 99 North 11th Avenue, Coatesville, PA 19320 - On or about March 7, 2025, the employer did not provide medical evaluations to employees who voluntarily wear respirators such as, but not limited to 3M 6300/07026 half facepiece respirators with P100 filters and Bullard clean air box model CAB30 with Bullard 20T hood, prior to the use of the respirators in the workplace.

29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:  a) Keehn Service Corporation located at or near 99 North 11th Avenue, Coatesville, PA 19320 - On or about February 25, 2025, the employer did not ensure that procedures for propane tank evacuation activities were clear and specifically outlined the steps to be followed in order to dissipate residual chemical energy. The procedure failed to clearly identify all of the specific steps to be followed by employees to control hazardous energy in the event of a visible frost line in the tank, thereby exposing employees to uncontrolled chemical energy.
