The employees were not protected by eye/face protection, on or about October 16th, 2024.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 55 Haladay Avenue, Yonkers - NY: Employees were exposed to fall hazards of approximately 12 1/2-feet while using an extension ladder to access a roof. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about, August 12th, 2024.
