29 CFR 1926.416(a)(3): Before work is begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work would bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer did not post and maintain proper warning signs where such a circuit exists. The employer did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken. a) On or about March 11, 2025, Sterrett Knob Masonry at the jobsite location of 120 N. Sandusky Ave. Upper Sandusky, OH: The employer failed to evaluate advise employee(s) of energized circuits prior to assignment of work. An employee operating from an aerial lift, performing masonry duties in proximity of energized lines without the benefit of identification and protective measures was exposed to electrical shock hazards.

29 CFR 1926.453(b)(2)(v):A body harness with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: a) On or about March 11, 2025, at the jobsite location of 120 N. Sandusky Ave. Upper Sandusky, OH; the employer Sterrett Knob Masonry, failed to ensure employees utilized fall protection while working from an elevated work platform. An employee working from a trailer mounted aerial lift, without the benefit of fall protection was exposed to falls over 25ft to the hard surface below.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section: a) On or about March 11, 2025, at the jobsite location of 120 N. Sandusky Ave. Upper Sandusky, OH; the employer Sterrett Knob Masonry, failed to implement engineering controls, work practices specified for the task in Table 1. Employees tasked with cutting brick and grinding mortar joints using a handheld power grinder without the benefit of dust collection vacuum system and/or water delivery system as specified in Table 1, were exposed to respiratory hazards including respirable crystalline silica.

29 CFR 1926.1153(i)(2)(i):The employer did not ensure that each employee covered by this section could demonstrate knowledge and understanding of the health hazards associated with exposure to respirable crystalline silica: a) On or about March 11, 2025, at the jobsite location of 120 N. Sandusky Ave. Upper Sandusky, OH; the employer Sterrett Knob Masonry, failed to implement a training program to assist employees in the recognition of hazards associated with respirable crystalline silica exposure. An employee performing brick cutting and mortar grinding without the benefit of this training was not equipped with the information necessary to recognize and mitigate the hazards associated with inhalation of silica dust.
