DBA Johnson Machine Company, 290 Bigler Avenue Clearfield, PA 16830 on or about 03/11/2025, and times prior thereto: The tagout signage used by employees at the Mattison Surface Grinder Machine #3 area was not substantial enough to prevent inadvertent or accidental removal. The tagout sign was made out of cardboard material and stated "Danger Lockout Tagout" and is placed behind the E-Stop of the machine.
