29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 11, 2025, at the location of a worksite located at 32345 Detroit Road, Avon, OH: The employer did not ensure that employees performing roofing activities on an 8/12 sloped roof were protected from fall hazards. Employees were exposed to fall hazards of 20 feet to the ground below while performing roofing activities.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 11, 2025, at the location of a worksite located at 32345 Detroit Road, Avon, OH:  employees were exposed to a fall hazard of approximately 8 feet while performing roofing activities on a residential home . Employees were using an extension ladder to access the roof, and the employer did not ensure they extended at least 3 feet above the upper landing surface. Two ladders were used in this manner, both on the front and rear of the building.
