On the roof at a residence, located at 3008 Lake Ellen Lane, Tampa, FL: On or about 03/12/2025, the employer exposed employees to struck by hazards, in that eye protection was not utilized by employees using compressed air powered nail guns while performing shingle installation.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On the roof located at 3008 Lake Ellen Lane, Tampa, FL. On or about 3/12/2025, the employer exposed employees to a fall hazard, in that the ladder did not extend at least three feet above the upper landing surface being accessed.
