On or about 3/12/2025, at 740 NW Enterprise Drive, Port Saint Lucie, Florida, the paint booth was not separated from other operations by at least 3 feet, thus exposing employees to a fire hazard while using the paint booth where flammable spray adhesive was used and had coinciding operations immediately outside of the spray booth.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:   On or about 3/12/2025 at 740 NW Enterprise Drive, Port Saint Lucie, Florida:  a. The LOTO program was deficient in that it did not have written procedures to LOTO all energy sources while an employee performed annual lubrication on the CNC machine (Weeke BMG111/Venture107M, SN - 0-250-17-2500), thus exposing an employee to caught-between hazards.  b.  The LOTO program was deficient in that it did not have written procedures to LOTO all energy sources while an employee performed annual lubrication on the Homag machine (Model - EDGETEQ S-380 Profiline, SN - 0-261-20-8822), thus exposing an employee to caught-between and amputation hazards.
