The LOTO program was deficient in that it did not have written procedures to LOTO all energy sources while an employee performed annual lubrication on the CNC machine (Weeke BMG111/Venture107M, SN - 0-250-17-2500), thus exposing an employee to caught-between hazards. b. The LOTO program was deficient in that it did not have written procedures to LOTO all energy sources while an employee performed annual lubrication on the Homag machine (Model - EDGETEQ S-380 Profiline, SN - 0-261-20-8822), thus exposing an employee to caught-between and amputation hazards.
