29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about March 13, 2025, located at 3809 15th Avenue, South Milwaukee, WI 53403, the employer had not ensured that employees performing tear off on the back side of the two-story residential property were protected from fall hazards. Exposing the employees to a fall hazard of approximately 17 feet onto a wood patio.

29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) On or about March 13, 2025, located at 3809 15th Avenue, South Milwaukee, WI 53403, the employer had not ensured employees were securing the metal snap hooks from the lifelines onto the back D-ring located between the employees shoulders while performing roofing work on an existing residential home. This exposed employees to a hazards associated with whiplash to the back from falling and suddenly stopping.

(a) On or about March 13, 2025, located at 3809 15th Avenue, South Milwaukee, WI 53403, the employer did not ensure the extension ladder used by employees to access and egress the residential roof was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 8 feet to the ground below.
