Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Located at 8565 Industrial Parkway, Plain City, OH 43064: On or about March 13, 2025, employees were exposed to fall hazards while walking and working from the top of the Building 80 Phase 6-Left Double Air Handler Unit (DAHU), approximately 18 feet above the concrete floor below. Employees were performing conduit installation work without fall protection.   b) Located at 8565 Industrial Parkway, Plain City, OH 43064: On or about March 13, 2025, and days prior, employees were exposed to fall hazards while crossing to access the mezzanine and while working from the top of the Building 80 Phase 6-Center Double Air Handler Units (DAHU), approximately 18 feet above the concrete floor below. The single personal fall arrest system provided did not provide multiple employees with the ability to maintain continuous fall protection across the approximately 14.5-foot by 29-foot unit.  c) Located at 8565 Industrial Parkway, Plain City, OH 43064: On or about March 13, 2025, and days prior, employees were exposed to fall hazards while walking and working from the top of the Building 71 Phase 10-Center Double Air Handler Units (DAHU), approximately 18 feet above the concrete floor below. The single personal fall arrest system provided did not provide multiple employees with the ability to maintain continuous fall protection across the approximately 14.5-foot by 29-foot unit.
