29 CFR  1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one of the methods listed in paragraphs 1910.28(b)(1)(i)(A) - (b)(1)(i)(C).  A) 161 Helen St South Plainfield, NJ 07080 - An employee, who engaged in loading products on the second level shelf, was not protected by proper fall protections while the elevation was approximately 10 feet. This condition exposed employees to fall hazards.  Incident Occurred on February 10, 2025.

29 CFR  1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  A) 161 Helen St South Plainfield, NJ 07080 - The employer failed to ensure each operator had been trained evaluated on powered industrial trucks. The employer failed to provide documentation which included the name of the operator, the date of the evaluation, and the identity of the person performing the evaluation. This condition exposed employees to struck by and fall hazards.   Hazards observed on or about February 11, 2025.
